I wanted to "cross post" this question from the Ambulatory Care Practitioners Forum to get the input from those of you working in the inpatient setting:

I'm wondering what tools, or methods, you use in your practices to help you perform medication reconciliation and to assess med adherence?  

It would be great to hear from those of you performing med rec in hospital settings as well, since many of us in the am care setting are working to try and improve the med rec process in the clinic setting!
