    Greetings AmCare Folks, and Happy Friday!

    For those of you practicing in ambulatory care practices, or providing MTM services in any setting, what types of billing methods are you utilizing (i.e. incident-to, third party payer contracts, Part D-specific mechanisms, transitions of care, Medicare annual wellness visits, chronic care management, other/etc)?

    We are doing TCM (transition care management) codes in our Internal Medicine Medical Home Clinics at Sanford Fargo with pharmacists and student pharmacists!

    That's great Maari!  Are you performing most of your TCM services via telephone, face-to-face, or combination of both?
    Since I am practicing in an FQHC setting, I'm not able to bill for my services due in part to pharmacists not currently being recognized federally as providers.  One avenue we are exploring, however, is the potential to bill for Medicare Annual Wellness Visits.  Many practices across the nation are currently doing this successfully--is there anyone in ND performing Annual Wellness Visits?
