Greetings,

I have a loaded question for all of you on this warm Friday afternoon!  I'm wondering what tools, or methods, you use in your practices to help you perform medication reconciliation and to assess med adherence?  

It would be great to hear from those of you performing med rec in hospital settings as well, since many of us in the am care setting are working to try and improve the med rec process in the clinic setting!
