    Recently, we started utilizing a heart failure smart list. This smart list helps us identify high-risk readmission patients for in-depth pharmacist review. I'm wondering if others throughout the state have smart list builds that allow pharmacists to prioritize review?

    How does the scoring work on that smart list? Which columns are being monitored and how are the patients pulled to the list?
