What is the process everyone is using for nursing home discharges.

Sanford Fargo is using an inter-facility transfer form.   We have recently pushed to do all discharge prescriptions e-prescribed to the Nursing Home pharmacies.  In the past, this was being done as a med print process.  Both have challenges.

MN has been working on a standard nursing home discharge process and form across all of their Epic sites.  What are your thoughts on this?
